Mike and Steve,
   The demo is up and running.
Printing works fine.
Customizing the form works fine.
Saving to the database works fine.

Remember, in IE set Internet Options/Advanced/Printing to "Print Background
colors and Images"

Recap of operation:
1.  First panel just shows dropdown list of treatments.  Select one and hit OK.
2.  Panel now shows 3 boxes with symptoms for that treatment.  THESE BOXES ARE
EDITABLE, as is the little box next to the OK button.
3.  To make the printer-friendly version, push the "make the form" button at the
bottom.  A new panel will appear with the font as large as will fit on one page.
 "File/Print Preview" from your browser menu to see what it will look like.
4.  To save your customized "Whoopdedo Surgery" form in the database, push the
"update the database" button at the bottom of the first panel (use the back button of your
browser if you need to get from the printer-friendly page to the active input
page).
5.  Restart the demo to get just the minimal starting panel.  When you click the
treatments pulldown, "Whoopdedo Surgery" will be on the list.  The format of the
stuff inserted into the database is somewhat messed up still, but it doesn't
hurt anything.
6.  To remove Whoopdedo Surgery from the database, manually delete all the text
in the 3 boxes (the little box next to the OK button still says "Whoopdedo
Surgery") and hit "update the database".
